The form is to be utilized when ever you issue yellow cards or red cards for Law 10 violations. Reminder Law 10 covers foul play.

Foul play is anything a player does within the playing enclosure that is against the letter and spirit of the Laws of the Game. It includes obstruction, unfair play, repeated infringements, dangerous play and misconduct which is prejudicial to the Game.

The form generates a record of your report and is immediately emailed to the competition chair and DC.

The last field “More information forthcoming” is to be used when you feel that the incident requires a conversation with the DC to discuss the incident in more detail. If you believe that the incident needs further discussion click the “yes” field and provide your cell phone number in the “other” field.
